In case of accidental death or permanent disability of the account holder farmer, any offspring of the account holder farmer as well as the spouse of the account holder farmer aged 5 to 70 years, they are eligible for the benefit of the scheme.
The main objective of the scheme is to provide financial assistance to the heirs of the account holder farmer, children of the account holder farmer as well as the husband or wife of the account holder farmer in case of accidental death or permanent disability. The Government of Gujarat has started a scheme from January 26, 1996 to provide insurance cover to the account holders of the state in case of accidental death or permanent disability. Under this scheme, insurance premium is paid by the state government on behalf of all the account holders of the state. This scheme has been implemented under Gujarat Collective Group Janata Accident Insurance Scheme since 2008 through the Director of Insurance, Gandhinagar.
All account holders holding land in individual or joint name, any child of the account holder farmer as well as in case of accidental death or permanent disability of the account holder's husband or wife aged 5 to 70 years are eligible for the scheme. The main terms of the scheme are as follows. The deceased or permanently disabled person who owns the land in individual or joint name should be the account holder farmer, the account holder farmer's child or the husband or wife of the account holder farmer. Death or permanent disability caused by accident. Suicide or natural death is not included in the plan. The age of the deceased or permanently disabled person should be 5 to 70 years. Application should be made to the office of the concerned District Agriculture Officer within 150 days.
The standard of assistance for this scheme is as follows.
* In case of death due to accident or permanent total disability, 100% of Rs. 2 lakhs
* In case of loss of two eyes or two limbs such as arms and legs due to an accident, Rs
* In case of loss of one eye and one limb, Rs. 2.00 lakhs
* 50% article Rs.1.00 lakh in case of loss of one eye or one limb due to accident
The documents required to attach the claim application of this scheme are as follows.
1. Application in prescribed form for accidental death or permanent disability insurance assistance
2. Appendix- 1,2,3, 3 (A), 4 and 5, seven bars, 8-A, village sample no.6 Certified extract after date of death
3. P.M. Report
4. FIR, Punchnama Report, Police
Inquiry Punchnama or Court Order
5. Death certificate of the deceased, proof of age
6. Report on Approval of Case by Subdivisional Magistrate
7. Certificate showing final assessment of medical board or civil surgeon in case of permanent complete disability and postcard size photograph showing disability
8. Valid driving license if the deceased is driving at the time of accident
9. Guarantee Form
10. Genealogy
11. Original genealogy of the heir in case the spouse is not an heir
12. Evidence sought by the Director of Insurance
The manner and time limit for applying in this scheme is as under. In case of accidental death, the heirs of the account holder farmer and in case of accidental disability, the account holder farmer should make an application to the concerned District Agriculture Officer, District Panchayat within 150 days from the date of death or accident. Applications received after 150 days will not be considered valid.
